To request a referral from TrueCare MD, the following criteria MUST be met per our practice policy:
- A patient visit must have taken place in the last 6 months
- The medical condition in which you are seeking a referral for should have been discussed in this visit to provide appropriate diagnosis and documentation for the referral
We will NOT complete any referral requests for any patients not seen within 6 months OR for any new medical condition as this requires an appointment with a Provider to determine medical necessity.
There are few ways you can request a referral with TrueCare MD and we ask that you allow a maximum of 10 business days to process referral requests:
- Utilize your patient portal to send a request to our team, we will send you confirmation via the patient portal of the request being finalized once we have completed the referral.
- Call our office at 602-777-7011, choose Option 3 (Internal Medicine Team) and then option 5 to speak to our clinical team so they can submit the request to our referral queue. You will receive a confirmation via your patient portal once we have completed the referral.
